The BHSF Medical Practice Ltd t/a BHSF Medical Practice will close as of 22 April 2020 and is no longer accepting new appointments/patients.
We still retain records in accordance with our retention policy/schedule and we take your privacy very seriously. Please read this privacy notice carefully as it contains important information on who we are and how and why we store, use and share existing personal information about you. It also explains your rights in relation to your personal information and how to contact us or the supervisory authority in the event you have a complaint.

Personal information we hold about you
We no longer collect personal information about you, but we do continue to store certain types of personal information depending on the relationship we had with you and the categories of personal information involved.
We may store the following personal information about you:
- Name and contact details such as address, telephone number email address and your next of kin details
- Date of birth, gender and nationality
- Your GP details and GP consent to share
- Medical history information including sample types, test results, vaccination info (including travel details), details of medication prescribed and any screening or referral records
- Your NHS number
What we use your personal information for
We use your personal information we have previously collected for storage purposes in accordance with our retention schedule. This is in order to keep an accurate record of any treatment you may have received from us, should you wish to gain access to such personal information in the future, or if any issues arise with regards any treatment you received. We may also use your personal information to administer the closure of the practice, to administer any of your rights you may wish to exercise or respond to any enquiries.
Who we share your personal information with
Your personal information is kept electronically on our cloud-based storage and we use third party providers to facilitate this cloud-based service. We may also share your personal data with other companies within the group or other third parties but only where it is necessary to administer your records. We may also share your personal information with law enforcement agencies and regulatory bodies to comply with any legal and regulatory obligations.
We may also need to share some personal information with other parties, such as potential buyers of some or all of the business or during a re-structuring. The recipient of the information will be bound by confidentiality obligations.
We only allow our service providers to handle your personal information if we are satisfied they take appropriate measures to protect your personal information. We also impose contractual obligations on service providers to ensure they can only use your personal information to provide the necessary services.
How long your personal information will be kept
We will keep your personal information for no longer than is necessary for the purpose for which it was collected and in accordance with our retention schedule. Different retention periods apply for different types of personal information and depending on the treatment you received. When it is no longer necessary to retain your personal information we will securely destroy/delete it.

You will not have to pay a fee to access your personal information (or to exercise any of the other rights). However, we may charge a reasonable fee if your request for access is manifestly unfounded or excessive. Alternatively, we may refuse to comply with the request in such circumstances.
